Logging an Accident
To log an accident on the system you will need to click on the accident and ill health module from the dashboard and then click on accidents.
To fill in the form you will need to click +create site record in the top right, as highlighted in the screen below. This will generate the form for you.
Pop in your name for person filling in the form.
You will need to answer from the drop down 'was anyone injured?' Answering this will determine whether you generate an accident or a near miss form.
If you select YES you will generate an Accident form.
If you select No you will generate an Near Miss form.
You will then need to fill in all the details of the accident that took place. If your accident form allows you to, you are able to log accidents that have taken place in different sites.
Please be aware that to submit the form, some specific fields have to be filled in - these fields have a red * next to them. If you try to complete the form with one field missing, the system will alert you of these specific fields and highlight in red necessary ones.
Once you have filled in all the details of the accident, you wish to add any supporting documents - photographs, witness statements etc. This can be done by the supporting files section, at the very bottom of the form. To add any files click +add files.
Select your document, fill in the description and click upload. To add more, keep selecting +add files.
You can preview any documents you have uploaded using the view files option.
When the form is completed scroll back up to the top of the form, click submit and then OK.
