Reviewing Health Surveillance Forms
Completed health surveillance forms that your staff have done are accessible via the health surveillance module on the site dashboard.
As this module contains sensitive data, you will need to make sure you have access to this module. This is not automatically given along with dashboard manager access and is a super user privilege.
Once you click on the module it will generate a list any completed questionnaires, along with any actions that have been raised as a result. To view a form select the ID number.
If a staff member has answers yes to any question it will automatically generate an action that will need to be closed off. They should input further information into the observation text field when completing the questionnaire so managers have all the information they need to address it. To address open actions raised which need attention you can click onto the red dot on a specific record
Alternatively you can click on the Action button when you have opened a form via the ID number.
Once in the action page you will have an overview of all issues which need to be addressed by the manager who can then edit/complete them or raise any additional actions as required.
When you select edit/complete the action will detail which question needs to be addressed. If the staff member has given additional information by free text typing in the observation box when selecting they have a health issue while completing their health surveillance form via their My Dashboard this will also show for convenience of the manager in the action field.
The target date represents when the action will appear red and overdue for managers in the system. Once a completed date is entered this will move the action into the completed section. We also advise to include who this was completed by and the action taken details.
When in the Health Surveillance Hub you have the option to view a matrix. This works similarly to the training matrix.
You have the option to view by dashboards or which order you want the information.
Additionally you can export this information to excel or select search to see an overview of the modules records.
