My Dashboard - Health Surveillance

To complete your health surveillance questionnaire, you will need to be on your dashboard and down to the quick links section. 

Click on health surveillance hub. 

If you are filling out this form for the first time please select create and confirm a new record creation.

Please read the questions carefully. If you answer yes, you are stating you have a health issue mentioned in the question. If you select yes an Observation box will appear. Here you need to note down any specific/additional information for your manager in relation to the question so they are aware of your answer. 

Once you have completed the form you must click submit. Your questionnaire will be sent to managers to review if required. 

If you do not Submit this form, none of the information will be saved and it will still appear as overdue in a 'draft' status and you will need to recomplete.
