Adding an Alleged Food Poisoning Form

To log an alleged food poisoning form, click on the module within the alleged food complaint module, as shown in the image below. 

To create a new form you will need to click  create site record.

PLEASE NOTE: 

When logging your complaint on Safety Cloud please ensure you are attaching your due diligence documents to the form. Once the complaint has been logged you will then need to send the customer a questionnaire to fill where they will confirm they wish to be contacted by your consultant

No contact will be made by your consultant until the customer has returned this questionnaire with consent. If you are having trouble locating this link, please email help@safetycloud.com .

All details of any investigation will be published in Safety Cloud, so please check any actions logged against the form for an update on progress. 

You will then need to fill in the form with all the details of the incident. 

You will need to attach the following records via the supporting files section at the bottom of the form:

  • Due diligence documents (temperature records and delivery, temperature checks for the day of the complaint and 3 days either side of the incident) 
  • Customer receipt
  • Covers sold of the dishes on the day and total covers for the service

You can add these documents via the + add files. 

Select a file from your computer, fill in a document description and click upload to start the upload. To add more files just keep clicking +add files. once you have completed the form click submit. 

Your consultant will make any notes on the investigation via the actions tab. You will receive an email every time the record is updated or if there is something you need to do, so please refer to the action section. 

When looking at the investigation history, you will need to note the number of coloured dots in the actions field. Any red dots will need to be actioned by the site.

Once the investigation has been completed, a consultant will add an action which will have an attachment with a formal response for you to forward onto customer. If the questionnaire has not been returned by the customer after two weeks, the case will be closed by your consultant. Any details will be added as actions on the form.
